Defining reminiscence therapy based on Rodgers' evolutionary concept analysis

Objective: To define the concept of reminiscence therapy through Rodgers' evolutionary concept analysis. Methods: China National Knowledge Infrastructure(CNKI), WanFang Database, VIP, China Biology Medicine Disc, PubMed, EMbase, CINAHL, and PsycINFO databases were retrieved.The concept analysis was conducted by using the Rodgers' evolutionary concept analysis as guiding framework. Results: A total of 34 literature published from 1963 to 2021 was included,whose main research areas were psychology,nursing,humanities, and social work.Reminiscence therapy had 7 related concepts(reminiscence,notalgia,autobiographical memory,oral history,narrative therapy,dignity therapy,life story work),3 attributes,and 3 antecedents.There were differences in the outcomes among different types of reminiscence therapy interventions. Conclusions: Clarifying relevant concepts,attributes,antecedents,and outcomes are beneficial for clinical practitioners to design and implement reminiscence therapy,reduce conceptual misuse,and lay the foundation for the development of standardized and innovative reminiscence therapy.

Prediction of cancer⁃specific survival status of patients with early colorectal adenocarcinoma after endoscopic therapy based on machine learning algorithms

Objective: To construct a cancer⁃specific survival status prediction model for patients with early colorectal adenocarcinoma after endoscopic treatment used machine learning algorithms. Methods: Based on SEER database,the data of 1 786 patients with early colorectal adenocarcinoma after endoscopic treatment were obtained,and the information included age,sex,race,cancer primary site,degree of cancer cell differentiation,pathological type of cancer tissue,radiotherapy,chemotherapy,tumor size,pathological condition,and marital status were extracted.After univariate Logistic regression and multivariate Logistic regression analysis,independent influencing factors of survival prognosis of patients with early colorectal adenocarcinoma after endoscopic treatment were determined.The patients were divided into training set and test set at a ratio of 8∶2.the factors with statistical differences in regression analysis were substituted into Logistic regression,random forest,extreme gradient boosting,support vector machine,decision tree,gradient boosting decision tree which were constructed by machine learning algorithm.To interpret results based on optimal machine learning models. Results: The results of multivariate Logistic regression showed that age,cancer primary site,degree of cancer cell differentiation,tumor size,pathological condition,and marital status were independent influencing factors of survival prognosis of patients with early colorectal adenocarcinoma after endoscopic treatment(<0.05).The area under the curve of receiver operator characteristic of random forest in the training set and test set were 0.876 and 0.858,respectively.And the F1 score were 0.791 and 0.739,respectively.The interpretability analysis of the model based on random forest showed that age,marital status and tumor size were more important,while higher age,larger tumor diameter,poor differentiation,and existence of submucosal infiltration were risk factors for death,and married were protective factors.Patients with the primary site of cancer in the right colon had poorer survival compared to those with the primary site of cancer in the left colon. Conclusions: The prognostic model constructed by machine learning for patients with colorectal cancer has good performance.It can provide accurate individualized prediction.

Visualization analysis of CiteSpace knowledge map of Chinese medicine nursing research on cardiac diseases in China

Objective: To analyze the status quo and hotspots of Chinese medicine nursing research on cardiac diseases in China by bibliometric methods. Methods: Literature related to Chinese medicine nursing research on cardiac diseases was retrieved from China National Knowledge Infrastructure,WanFang Database,China Biology Medicine database,and VIP database.Literature included was used for visual analysis. Results: The overall volume of Chinese medicine nursing research on cardiac diseases showed upward trend.However,the collaboration among authors and research institutions was insufficient.The hotspots symptoms studied mainly were constipation,anxiety,depression,and insomnia and so on.The research field of Chinese medicine nursing research on cardiac diseases paid more attention to the elderly,advocates dialectical nursing and comfortable nursing, with the goal of improving the quality of life of patients. Conclusions: The quantity of Chinese medicine nursing research on cardiac diseases is less.The collaboration among authors and research institutions needs improvement.Chinese medicine nursing plans on cardiac diseases need to be optimized,so sa to providing reference for clinical nurses to carry out dialectical nursing and implement appropriate Chinese medicine nursing techniques,and promoting the long⁃term and stable development of Chinese medicine nursing.

Construction and application of breastfeeding responsive feeding intervention program for infants based on self⁃determination theory

Objective: To construct an breastfeeding responsive feeding intervention program for infants based on self⁃determination theory and preliminarily to explore its application effects. Methods: 50 parturients who gave birth in a tertiary hospital were selected as subjects by convenient sampling methods from February 1,2023 to May 31.The parturients were divided into intervention group and control group,with 25 cases in each group.Based on self⁃determination theory,a breastfeeding responsive feeding intervention program for infants was constructed through literature review,group discussion,and Delphi expert inquiry.The control group received routine nursing measures implemented by the hospital's obstetrics department,while the intervention group implemented breastfeeding responsive feeding intervention program for infants on the basis of the control group.The parturients knowledge level of responsive feeding,breastfeeding self⁃efficacy,breastfeeding motivation,pure breastfeeding rate,weight and length of infant were compared between the two groups. Results: During the construction process of breastfeeding responsive feeding intervention program for infants,the response rates of the 2 rounds of expert inquiry questionnaires were both 100%,the authority coefficients were 0.92 and 0.94,the coefficient of variation ranged from 0.00 to 0.29 and ranged from 0.05 to 0.20,the Kendall coefficient of concordance were 0.214 and 0.165(both <0.001).After intervention,the score of parturients knowledge level of responsive feeding in intervention group was (16.80±0.50)points,the score of breastfeeding self⁃efficacy was (58.16±9.33)points,the score of self⁃motivation was (61.00±5.79)points,the score of controlled motivation was(29.08±3.27)points,all of which were higher than the control group,the score of parturients knowledge level of responsive feeding in control group was (13.56±2.35)points,the score of breastfeeding self-efficacy was (44.84±8.60)points,the score of self-motivation was (51.04±8.36)points,the score of controlled motivation was(24.00±3.81)points.The pure breastfeeding rate of intervention group were higher than those of the control group on the day of discharge,one week after discharge,and 42 days postpartum. Conclusions: The breastfeeding responsive feeding intervention program for infants constructed in this study is scientific and reliable,which is conducive to improving parturients knowledge level of responsive feeding,breastfeeding self⁃efficacy,it can also help parturients to establish breastfeeding motivation and improve pure breastfeeding rate.

Construction and validation of risk prediction model of adverse blood transfusion reaction after allogeneic hematopoietic stem cell transplantation

Objective: To explore the risk factors of adverse blood transfusion reaction after allogeneic hematopoietic stem cell transplantation(allo⁃HSCT),establish a risk prediction model and validate it. Methods: A total of 115 allo⁃HSCT patients in our hospital were selected as the study subjects from January 2018 to February 2023,then they were divided into modeling group(73 cases) and validation group(42 cases).The general information of the patient was investigated,C⁃reactive protein(CRP),albumin(ALB),neutrophils,lymphocytes in the patient's serum were tested,and CRP/ALB and neutrophil/lymphocyte ratio(NLR) were calculated.Single factor analysis and Logistic regression analysis were used to screen the risk factors of allo⁃HSCT adverse blood transfusion reaction,risk prediction model was constructed and transformed into risk scoring system.The differentiation and calibration of the evaluation model were conducted by receiver operating characteristic(ROC) curve and the Hosmer⁃Lemeshow(H⁃L) test,and the model was verified. Results: The levels of CRP/ALB and NLR in adverse blood transfusion reaction group were higher than those in without adverse blood transfusion reaction group(all <0.05).Logistic regression analysis showed that the number of blood transfusio≥3 times,had the history of primary blood disease,had blood transfusion history,had allergy history,the patient's basic temperature≥38 ℃,the time from blood distribution to blood transfusion≥30 min,infusion rate of red blood cells≥ 50 drops per minute,platelet infusion rate ≥90 drops per minute,CRP/ALB≥0.90,and NLR≥1.37 were risk factors for allo⁃HSCT adverse blood transfusion reaction.The area under the curve(AUC) of receiver operator characteristic of the model group was 0.841, and the H⁃L test result showed =0.856.The sensitivity of the model was 0.909,the specificity was 0.775,and the Youden index was 0.684.The area under the curve of the validation group was 0.798,the H⁃L test result was =0.813,the sensitivity was 0.818,the specificity was 0.775,and the Youden index was 0.593. Conclusions: The risk prediction model of allo⁃HSCT adverse blood transfusion reaction constructed in this study has high predictive effect and can provide targeted guidance for the nursing of allo⁃HSCT adverse blood transfusion reaction in clinical practice.

Status quo and influencing factors of oral health of ICU patients with invasive mechanical ventilation from tertiary hospitals in the Guangxi Zhuang Autonomous Region

Objective: To investigate the status quo of oral health of ICU patients with invasive mechanical ventilation from tertiary hospitals in Guangxi Zhuang Autonomous Region,and to analyze its influencing factors. Methods: A total of 204 ICU patients with invasive mechanical ventilation from 5 tertiary hospitals in Guangxi Zhuang Autonomous Region were selected as the research subjects by convenience sampling method in July 2023.The general information questionnaire and Beck Oral Assessment Scale(BOAS) were used for the survey. Results: The BOAS score of ICU patients with invasive mechanical ventilation was (7.79±2.86)points,and 62 patients(30.39%) had no damage to oral health.The results of the random forest model showed that the 5 variables with the higher importance score were the duration of mechanical ventilation,whether to use specialized mouthwash or not,Acute Physiology and Chronic Health Evaluation Score Ⅱ(APACHE Ⅱ) score,whether to use a toothbrush or not,and whether to moisturize the lips or not. The results of stepwise regression analysis showed that APACHE Ⅱ score,whether to use a toothbrush or not,whether to use specialized mouthwash or not,and whether to moisturize the lips or not were the main influencing factors on the oral health of ICU patients with invasive mechanical ventilation(<0.05). Conclusions: The proportion of ICU patients with invasive mechanical ventilation who suffer from oral health damage is relatively high,and it is influenced by multiple factors.It is recommended to adopt comprehensive intervention measures to promote the oral health of patients.

Construction of nursing intervention plan for patients with calciphylaxis based on Delphi method

Objective: To construct a nursing intervention plan for patients with calciphylaxis based on Delphi method. Methods: A preliminary nursing intervention plan for patients with calciphylaxis was developed through literature search and research group discussion.Adopting the Delphi expert inquiry method for expert inquiry,the final draft of nursing intervention plan for patients with calciphylaxis was formed. Results: 2 rounds of expert inquiry were conducted,and the effective response rate of the questionnaire were both 100%,the authority coefficients were 0.866 and 0.906,the coefficient of variation ranged from 0.06 to 0.24 and ranged from 0.09 to 0.23,and the Kendall coefficient of concordance were 0.111 and 0.146(<0.001).The final constructed nursing intervention plan for patients with calciphylaxis included 7 first⁃level items,22 second-level items,and 55 third-level items. Conclusions: The nursing intervention plan for patients with calciphylaxis constructed in this study is reliable and practical.It can provide reference for nursing intervention for patients with calciphylaxis.

Construction of early warning grading criteria and intervention plan for monitoring dynamic intra⁃abdominal pressure in patients with severe acute pancreatitis undergoing enteral nutrition

Objective: To construct early warning grading criteria and intervention plan for monitoring dynamic intra⁃abdominal pressure in patients with severe acute pancreatitis(SAP) undergoing enteral nutrition. Methods: Retrieve China and foreign literature,the draft of early warning grading criteria and intervention plan for monitoring dynamic intra⁃abdominal pressure in SAP patients undergoing enteral nutrition was drafted.2 rounds of inquiry with experts were conducted by Delphi expert inquiry method,and the early warning grading criteria and intervention plan for monitoring dynamic intra⁃abdominal pressure in SAP patients undergoing enteral nutrition were formed. Results: The questionnaire response rates of 2 rounds of expert consultation both were 100%,the authority coefficient was 0.918,the coefficient of variation ranged from 0.077 to 0.156 and ranged from 0.056 to 0.167,respectively.The Kendall coordination coefficients were 0.353 and 0.331(<0.001),respectively.The final constructed early warning grading criteria and intervention plan for monitoring dynamic intra⁃abdominal pressure in SAP patients undergoing enteral nutrition included 3 first⁃level indicators,13 second⁃level indicators,and 26 third⁃level indicators. Conclusions: The early warning grading criteria and intervention plan for monitoring dynamic intra⁃abdominal pressure in SAP patients undergoing enteral nutrition constructed in this study have certain scientific and practical significance,which is beneficial for guiding clinical practice.

Construction and validation of risk prediction model of dysarthria in patients with stroke

Objective: To explore the risk prediction model of dysarthria in patients with stroke,construct risk prediction model and validate their effectiveness. Methods: Convenience sampling was used to select stroke patients from 4 tertiary hospitals in Hebei province from July to November 2022.Patients in case group were 173 cases and control group were 316 cases.A general information questionnaire,oral feature questionnaire,Chinese version of Brief Oral Health Status Examination,National Institutes of Health Stroke Scale,and the Chinese version of Montreal Cognitive Assessment Scale were used to survey patients,and Lovett muscle strength grading and water swallow test were used to evaluate patients. Results: Age≥60 years old,brainstem stroke,history of stroke,poor tongue and body movement,moderate and severe neurological dysfunction,poor oral health,cognitive and swallowing dysfunction were risk factors of dysarthria in patients with stroke(<0.05). The area under the curve of receiver operator characteristic of predictive model based on risk factors was 0.837[95%CI(0.798,0.876)],the sensitivity was 0.905,the specificity was 0.642,Brier score calculated by Bootstrap internal validation was 0.148,H⁃L test showed =0.177,the decision curve showed high risk probability ranged from 0.08 to 0.10,and the C⁃index of the Nomogram was 0.837,=0.743. Conclusions: Medical staff should focus on the age,stroke history,stroke location,degree of neurological dysfunction,tongue and oral health,cognitive and swallowing functions of stroke patients.The predictive model constructed in this study can be used to evaluate patients and develop targeted measures early.

Moderating effect of inclusive leadership on nurse conflicts and nursing team work atmosphere

Objective: To explore the relationship between nurse conflict,service atmosphere,and safety atmosphere,as well as the moderating effect of inclusive leadership. Methods: A total of 1 509 nurses from 5 hospitals in Yangquan city were selected as the research subjects in March 2023.Nurse general information questionnaire, Nurse⁃Nurse Relationship Scale,Service Climate Scale,Safety Climate Scale,and Inclusive Leadership Scale were used for surveys. Results: The conflict and contradiction score of 1 509 nurses was (3.37±0.92)points,the service climate score was (5.15±1.00)points,the safety climate score was (5.52±1.16)points,and the inclusive leadership score was (5.05±1.18)points.The correlation analysis results showed that nurse conflicts were negatively correlated with both service climate and safety climate(<0.05).The results of linear stratified regression analysis showed that inclusive leadership had moderating effect between nurse conflict and service climate,it also had moderating effect between nurse conflict and safety climate(<0.05). Conclusions: Nurse conflicts have a negative impact on service climate and safety atmosphere.Inclusive leadership can mitigate this negative impact.

Dose⁃response relationship between sleep duration and depressive symptoms among empty⁃nesters in China

Objective: To explore the dose⁃response relationship between sleep duration and depressive symptoms among empty⁃nesters in China. Methods: A total of 2 321 empty⁃nesters aged 60 and above from the 2018 China Health and Retirement Longitudinal Survey were selected as the study subjects.Logistic regression and restricted cubic spline(RCS) models were used to analyze the dose⁃response relationship between sleep duration and depressive symptoms. Results: 918 out of 2 321 empty⁃nesters were diagnosed with depressive symptoms,with detection rate of 39.55%.The detection rate of depressive symptoms in males was 31.25% ,and in females was 49.81%.The detection rate of depressive symptoms in empty⁃nesters with sleep duration less than 5 hours was 62.47%,the detection rate of depressive symptoms in empty⁃nesters with sleep duration ranged from 5 to less than 7 hours was 41.15%,the detection rate of depressive symptoms in empty⁃nesters with sleep duration ranged from 7 to less than 9 hours was 29.55%,the detection rate of depressive symptoms in empty⁃nesters with sleep duration ranged from 9 to less than 11 hours was 33.81%,and detection rate of depressive symptoms in empty⁃nesters with sleep duration≥11 hours was 33.98%.The results of multivariate Logistic regression analysis showed that after adjusting for confounding factors,the detection rate of depressive symptoms in patients with sleep duration<5 hours and ranged from 5 to less than 7 hours were 2.529 times and 1.549 times higher than that in patients with sleep duration ranged from 7 to less than 9 hours,respectively(<0.05).The RCS model analysis results showed that both overall analysis and gender stratification showed non⁃linear dose⁃response relationship between sleep duration and depressive symptoms among empty⁃nesters(overall trend <0.01,non⁃linear test <0.01). Conclusions: Insufficient sleep duration is a risk factor for depressive symptoms among empty⁃nesters.It is recommended to strengthen family and social support,so as to improve the sleep environment of empty⁃nesters,and reduce the risk of depressive symptoms among empty⁃nesters.

Investigation on coping strategies for the increases of patients from emergency department in tertiary hospitals in China

Objective: To understand coping strategies for the increases of patients from emergency department in tertiary hospitals in China. Methods: Emergency departments from 54 tertiary hospitals in China were selected as the research subjects From January to March 2023.Among them,26 hospitals(48.1%) in the eastern region,10 hospitals(18.5%) in the central region,and 18 hospitals(33.3%) in the western region.A survey was conducted by a self⁃designed questionnaire on coping strategies for the increases of patients from emergency department in hospitals in different regions of China. Results: After increasing of patients from emergency department in tertiary hospitals,the daily average emergency volume increased by 87.5%,and the overall waiting time for emergency treatment was extended(=-5.560,<0.001).The proportion of patients leaving the emergency room 72 hours after increased of patients from emergency department in tertiary hospitals in the eastern region decreased compared to before(<0.05).In the context of increases of patients from emergency department,the response measures taken by the tertiary hospitals included increasing the number of emergency beds and the number of visiting doctors,adding wards dedicated to the treatment of patients with COVID⁃19,increasing investments in important equipment,and deploying medical staff to support emergency and so on.The change rate of emergency waiting time in tertiary hospitals in eastern region were negatively correlated with the change rate of doctor⁃patient ratio(=-0.418,=0.034),the change rate of nurse⁃patient ratio(=-0.469,=0.016),and the change rate of Internet diagnosis and treatment(=-0.684,=0.020).The change rate of retention time in emergency rooms in tertiary hospitals in eastern region was negatively correlated with the change rate of nurse-patient ratio(=-0.422, =0.032).The change rate of retention time in emergency rooms in tertiary hospitals in central and western regions was positively correlated with the change rate of observation room beds(=0.405,=0.036). Conclusions: The emergency department should constantly optimize the allocation of medical resources such as manpower,beds,instruments and equipments,it also should explore the new "Internet+" model for critical patients,and further improve the ability to cope with major infectious diseases.
