Construction of predictive model of frailty risk in elderly patients with chronic heart failure based on machine learning

Objective: To construct a predictive model of frailty risk in elderly patients with chronic heart failure(CHF) based on machine learning,and to provide a new method for accurate prediction of frailty occurrence in clinical elderly patients with CHF. Methods: Clinical data related to CHF patients from the cardiovascular medicine department of a tertiary grade A hospital in Urumqi from January 2023 to May 2023 were collected and randomly divided into training and testing sets in the ratio of 7∶3,with the occurrence of frailty as the outcome variable.The frailty risk prediction models were constructed based on four algorithms:Logistic regression(LR),decision tree(DT),random forest(RF),and support vector machines (SVM).The performance of the models was evaluated based on the area under curve (AUC),accuracy,precision,sensitivity,specificity,F1 value,and the optimal model was selected. Results: A total of 423 patients with CHF were included,182 of whom developed frailty(43%).All four prediction models had high accuracy,and the AUC values of the LR,DT,SVM,and RF models were 0.917,0.863,0.941 and 0.952,respectively,with the RF models having the highest AUC values,and the RF model had the highest accuracy,precision,sensitivity,specificity,and F1 value were the highest.The importance of the feature variables was further ranked based on the RF model,and the top five feature variables were hemoglobin,interleukin⁃6,albumin,malnutrition,and Charlson Comorbidity Index(CCI) scores. Conclusion: The predictive model of frailty risk in elderly patients with chronic heart failure based on RF machine learning has the best performance,which is helpful for early clinical assessment and prevention of frailty risk.

Chinese version of the Relocation Stress Syndrome Scale⁃Short Form and its reliability and validity test

Objective: To translate the Relocation Stress Syndrome Scale⁃Short Form(RSSs⁃SF) into Chinese,and to test its reliability and validity. Methods: The English version of RSSs⁃SF was translated,back⁃translated according to the Brislin model,and the Chinese version of RSSs⁃SF was formed after cultural adaptation and pre⁃surveyed.A total of 220 patients who admitted to the department of ICU in 3 tertiary grade A hospitals in Shiyan city were investigated to test the reliability and validity of the scale. Results: The Chinese version of RSSs⁃SF included 3 dimensions and 10 items.The Cronbach´s α coefficient of the scale was 0.867,and the Cronbach's α coefficients of each dimension ranged from 0.833 to 0.876.The Guttman split⁃half coefficient of the total scale was 0.729,and the Guttman split⁃half coefficients of each dimension ranged from 0.750 to 0.844.The test⁃retest reliability was 0.906.The scale⁃level content validity index(S⁃CVI)was 0.933.The item⁃level content validity index(I⁃CVI) ranged from 0.833 to 1.000.Exploratory factor analysis extracted 3 common factors,and the cumulative contribution rate of variance was 74.686%.The results of confirmatory factor analysis show that the three⁃factor model was well⁃fitted. Conclusion: The Chinese version of the RSSs⁃SF has good reliability and validity,and it can be used as an assessment tool for relocation stress in domestic department conversion patients in China.

Chinese version of Patient Participation Scale and its reliability and validity test

Objective: To translate the Patient Participation Scale(PPS) into Chinese,and to test its reliability and validity. Methods: The scale was translated according to the Brislin translation model,and the Chinese version of PPS was developed through translated,back translation,expert consultation and pre⁃investigated.A survey was conducted among the outpatient patients or inpatients in a tertiary grade A hospital in Tianjin from March to April 2023,and test the reliability and validity of the scale, Results: The Chinese version of PPS included 4 dimensions and 21 items.The overall Cronbach's α coefficient was 0.851,the retest reliability was 0.731,and the half reliability was 0.690.The item⁃level content validity index(I⁃CVI)ranged from 0.857 to 1.000.The scale⁃content validity index(S⁃CVI) was 0.904.Exploratory factor analysis extracted 4 common factors,and the cumulative contribution rate of variance was 67.788%. Conclusion: The Chinese version of PPS has good reliability and validity,and it can be used to evaluate the degree of patient participation.

Status quo and influencing factors of telecare readiness for community nurses

Objective: To investigate the status quo of telecare readiness for nurses in community health service center,and to analyze its influencing factors,so as to provide references for implement telecare in communities. Methods: A total of 462 nurses from 35 community health service centers in Shandong province were selected by convenience sampling.A cross⁃sectional study was conducted by the general data questionnaire,Telehealth Readiness Assessment Tools(TRAT),Nursing Informatics Competency Assessment Scale,Narrative Competence Scale(NCS) and the Job Crafting Scale. Results: The total score of the community health service center nurses for telecare readiness was 56.00(51.00,64.00).The results of multiple linear regression analysis showed that whether willing to provide telecare for patients,nursing informatics competency,narrative competence and the job crafting were the main influencing factors of telecare readiness(<0.01). Conclusion: Nurses in community health service center have a low level of telecare readiness.It is urgent to formulate effective intervention strategies based on its influencing factors,such as improving the incentive mechanism and training system,ensuring the accessibility of community resources and information and appropriate empowerment,so as to improve the quality of telecare personnel and promote the comprehensive development of telecare.

Chinese version of Productive Engagement Scale and its reliability and validity test

Objective: To translate the Productive Engagement Scale into Chinese,and to test its reliability and validity. Methods: Translation and back⁃translation of the Productive Engagement Scale were conducted following the Brislin model and Guidelines for the Cross⁃Cultural Adaptation by the American Association of Orthopaedic Surgeons(AAOS) Outcomes Committee,then expert consultation and pilot survey were also used to culturally adapt the scale.Finally,429 community⁃living older people were surveyed to test the reliability and validity. Results: The Chinese version of Productive Engagement Scale included 1 dimension and 9 items.The item⁃content validity index(I⁃CVI) ranged from 0.875 to 1.000,and the scale⁃content validity index(S⁃CVI) was 0.890.The Cronbach's alpha coefficient,test⁃retest reliability,and split⁃half reliability were 0.923,0.865 and 0.826,respectively.Confirmatory factor analysis showed that the model fit the data well. Conclusion: The Chinese version of Productive Engagement Scale has good reliability and validity,which provides a measurement tool for assessing productive engagement in community⁃living older people.

Chinese version of Communication Behavior Scale for Nurses Caring for People with Dementia and its reliability and validity test

Objective: To translate the Communication Behavior Scale for Nurses Caring for People with Dementia (CBS⁃D),and to test its reliability and validity among elderly care workers. Methods: The Brislin model was adopted to conduct literal translation,retranslation and cultural adaptation of CBS⁃D.The reliability and validity of the Chinese version scale were tested among 206 elderly care workers recruited by convenience sampling. Results: The Chinese version of CBS⁃D had 4 dimensions and 18 items.The average of scale level content validity index(S⁃CVI/Ave) was 0.96,and the item level content validity index(I⁃CVI) ranged from 0.80 to 1.00.The results of confirmatory factor analysis showed that the model was well⁃fitted.The total Cronbach's α coefficient of the Chinese version scale was 0.904,and the Cronbach's α coefficients of each dimension ranged from 0.841 to 0.861.The test⁃retest reliability was 0.900.And the half⁃reliability was 0.760. Conclusion: The Chinese version of the CBS⁃D has good reliability and validity,and it can be used as an evaluation tool for the communication behavior of elderly care workers.
