Conflict analysis of endowment resource allocation for rural left⁃behind elderly based on GMCR⁃AHP

Objective: To analyze the interest relationship between different subjects involved in the process of supporting the left⁃behind elderly in rural areas,and to provide strategic suggestions for improving the reasonable allocation of rural supporting resources. Methods: Analytic hierarchy process(AHP) and graph model for conflict resolution(GMCR) were used to construct the GMCR⁃AHP model of the interest demands of the government,pension institutions,families and the elderly,analyze and optimize the conflicts among the relevant subjects in the current allocation of pension resources for the left⁃behind elderly in rural areas,and explore the global stable state of the interest balance of each subject. Results: Through conflict analysis,the equilibrium solution is YNYNNYNY,that is,the government improves supporting policies and leads the old⁃age care work.Old-age care institutions strive for more government subsidies to expand the range of old⁃age care services;children in the family can not take care of the elderly,choose institutions for the aged;the elderly themselves are eager to get more government economic policy support to ease the pressure of family pension. Conclusions: In order to alleviate the conflict of interests of various subjects under the dilemma of supply and demand of rural pension resources and rationally allocate rural pension resources,the government should lead the cooperative governance of multiple subjects and increase the investment in rural pension industry.Pension institutions should keep up with the change of policy direction and develop the service mode of combining medical and nursing care;Families take the health of the elderly as the first priority and support institutions for the left⁃behind elderly.At the same time,the old people themselves change the traditional pension concept and actively participate in the pension mode under the new situation.

Study on home⁃based care service preferences of disabled elderly people in Beijing based on discrete choice experiments

Objective: The aim of this study is to explore the preferences of disabled elderly people for home-based medical care services in Beijing,with a view to enhancing the utilization rate of said services. Methods: Utilizing a discrete choice experimental design and aided by a self⁃designed questionnaire,convenient sampling was conducted for face⁃to⁃face surveys among 253 elderly individuals and their caregivers in two Beijing communities.Preferences for home⁃based medical care services among the disabled elderly were determined using a mixed logit model. Results: Significant influences(<0.05) were found on preferences for home⁃based care among the disabled elderly⁃aspects like payment costs,institutional nature,appointment method,and service content all playing a part.The order of attributes based on willingness to pay are:institutional nature,appointment method,service content and service decisions.Factors including gender,education level,disability degree,income,number of children,and marital status all affect the home care service preferences of disabled elderly individuals. Conclusion: Home⁃based disabled elderly individuals prefer lower-cost services from public institutions that can be scheduled online or offline.They prefer not to receive in-home health management services and show no preference towards service decisions or caregiver experience.These findings can be used to cater services better to the preferences and specific characteristics of home-based disabled elderly individuals.

Construction and validation of a risk prediction model for body image disturbance in patients with head and neck cancer

Objective: Analyze the risk factors for the development of body image disturbance in head and neck cancer patients,construct a nomogram graph prediction model,and validate the predictive effect of the model. Methods: A total of 332 head and neck cancer patients hospitalized in the Department of otolaryngology,head and neck surgery,stomatology,and radiotherapy of A tertiary hospital in Taiyuan from June to November 2022 and hospitalized in the Department of Head and Neck Surgery of a specialized oncology hospital in Taiyuan from July to September 2022 were selected as study subjects.The patients were divided into the modeling group(235 cases) and the validation group(97 cases) according to the ratio of 7∶3.Based on whether body image disturbance occurred after treatment,the patients in the modeling group were divided into body image disturbance group and normal body image group,and the risk factors of body image disturbance were explored by logistic regression analysis,prediction model were established,nomogram were drawn,and the discrimination and calibration of the model were evaluated by area under the ROC curve and Hosmer⁃Lemeshow test,respectively. Results: The incidence of body image disturbance was 37.05% in 332 patients with head and neck cancer;a prediction model was constructed based on six influencing factors:tumor stage,presence of tracheal cannula,functional status score,anxiety score,comprehension social support score,and negative coping score.The area under the ROC curve of the model was 0.897(<0.001) and 0.850(<0.001) in the modeling and validation groups,respectively,with good discrimination; the Hosmer⁃Lemeshow values were 7.828(=0.348) and 5.409(=0.610),respectively,with good calibration. Conclusion: The study constructed a nomogram graph prediction model of body image disturbance in head and neck cancer patients with good prediction effect,which can provide reference for clinical nurses to assess the risk of body image disturbance in head and neck cancer patients and make early intervention.

Latent profile analysis of irrational parenthood cognitions among female undergoing in vitro fertilization⁃embryo transfer

Objective: This article aims to classify irrational parenthood cognitions characteristics among females undergoing in vitro fertilization⁃embryo transfer(IVF⁃ET),and to explore the characteristics of various types of irrational parenthood cognitions among patients. Methods: From October 2022 to January 2023,a total of 449 females undergoing IVF⁃ET from a tertiary first⁃class hospital in Hangzhou city were enrolled as investigation subjects via convenience sampling methodology.General Information Questionnaire and Irrational Parenthood Cognitions Questionnaire(IPCQ) were utilized as the investigating tools. Results: The irrational parenthood cognitions among females undergoing IVF⁃ET could be divided into 3 classes,namely low irrational parenthood cognitions group(24.3%),moderate irrational parenthood cognitions group(54.8%),and high irrational parenthood cognitions group(20.9%).The results of multiple logistic regression showed that family income,education level and duration of infertility were influencing factors regarding to irrational parenthood cognitions among female undergoing IVF⁃ET. Conclusions: The irrational parenthood cognitions among females undergoing IVF⁃ET had apparent classification characteristics.Clinical workers should give targeted psychological and information support according to the characteristics of patients,so as to reduce the level of irrational parenthood cognitions.

Construction and validation of a risk prediction model for hyperuricemia in children and adolescents

Objective: To investigate the risk factors for the development of hyperuricemia in children and adolescents and to construct a risk prediction model. Methods: The clinical data of 9 857 cases of children and adolescents aged 6~17 years attending the Affiliated Hospital of Chuanbei Medical College from January 2017 to December 2021 were retrospectively analyzed,and 8 689 cases from January 2017 to December 2020 constituted the modeling group,and 1 168 cases from January to December 2021 constituted the validation group for internal validation.Referring to the definition of hyperuricemia,they were divided into hyperuricemia group and non⁃hyperuricemia group.Univariate analysis and Logistic regression were applied to analyze the influencing factors of hyperuricemia in children and adolescents,and prediction models were developed.The area under the subject operating characteristic(ROC) curve(AUC) and C⁃index were used to assess the predictive ability of the model,and Bootstrap repeated sampling method(1 000 times of sampling) was used for internal validation of the model. Results: The results of multifactorial analysis showed that age,gender,body mass index,creatinine,urea nitrogen,estimated glomerular filtration rate,triglycerides,high⁃density lipoprotein as influencing factors of hyperuricemia in children and adolescents.The area under the ROC curve AUC of the model was 0.876,the sensitivity was 0.809,the specificity was 0.796,the Youden index was 0.611,and the C⁃index was 0.877.the area under the ROC curve AUC of external validation was 0.838,and the overall correctness of the model was 77.8%. Conclusion: The risk prediction model for hyperuricemia in children and adolescents has good predictive efficacy and implementability,and provides a reference for clinical staff to assess the risk of hyperuricemia in children and adolescents.

Relationship between ultrasound parameters and body mass index, bone mineral density and skeletal muscle mass index in patients with sarcopenia

Objective: To explore the relationship between ultrasound parameters and body mass index(BMI),bone mineral density(BMD) and skeletal muscle mass index(SMMI) in patients with sarcopenia. Methods: 60 patients with sarcopenia in Second Hospital of Shanxi Medical University were regarded as study group from January 1,2023 to January 1,2024,and 50 healthy volunteers with gender and age matching who came to our hospital for physical examination during the same period were selected as control group.Ultrasound examination was performed in both groups to record gastrocnemius muscle thickness(MT),maximum cross⁃sectional area(CA) and muscle hardness(Young' s modulus values during contraction and relaxation).Dual⁃energy X⁃ray absorption method was used to record BMD and limb muscle mass and calculate the SMMI and BMI.The differences in the above indicators were compared.Logistic regression analysis was used to screen the influencing factors of sarcopenia.ROC curve was applied to analyze the diagnostic efficiency of ultrasound parameters on sarcopenia.Pearson correlation coefficient was adopted to analyze the correlation between ultrasound parameters and BMI,BMD and SMMI in study group. Results: Logistic regression analysis showed that MT,CA,Young's modulus values during contraction and relaxation,BMI,BMD and SMMI were independent influencing factors of sarcopenia(<0.05).ROC curve revealed that the AUCs of MT,CA,Young' s modulus values during contraction and relaxation in the diagnosis of sarcopenia were 0.908,0.958,0.909,0.932 respectively.Pearson correlation coefficient manifested that MT,CA and Young' s modulus values during contraction and relaxation were positively correlated with BMI,BMD and SMMI in patients with sarcopenia(<0.05). Conclusion: Ultrasound parameters are helpful for screening and diagnosis of sarcopenia.Ultrasound parameters MT,CA and muscle hardness are significantly correlated with BMI,BMD and SMMI.

Development,reliability and validity test of pregnancy associated venous thromboembolism risk assessment scale

Objective: To develop a risk assessment scale for maternal pregnancy⁃related venous thrombosis in China,and to verify its reliability and validity. Methods: The pre⁃test scale was formed by literature research and two rounds of expert evaluation.It is convenient to select 202 pregnant women for investigation,and carry out item analysis and reliability and validity test of the pre⁃test scale. Results: The pregnancy⁃related venous thrombosis risk assessment scale contained 5 dimensions and 16 items,and a total of 6 common factors were extracted by exploratory factor analysis.The cumulative variance contribution rate was 60.832%,the area under ROC curve was 0.836,the Cronbach's α coefficient of the scale was 0.852,and the total retest reliability was 0.920 after two weeks. Conclusion: The pregnancy-related venous thrombosis risk assessment scale developed in this study has good reliability and validity,and can effectively evaluate the risk coefficient of pregnancy⁃related venous thrombosis in pregnant women.

Study on relationship between dietary pattern,inflammatory index and metabolic index of chronic diseases

Objective: To investigate the relationship between dietary pattern,dietary inflammation index(DII) and chronic metabolic indexes of adult residents in Shanxi Province. Methods: A total of 1 396 adult residents were included in the study using multi-stage stratified cluster random sampling method based on the data of China Nutrition and Health Monitoring(CNNHS) project in Shanxi Province from 2010 to 2012.Through questionnaire survey,physical examination and laboratory examination,different dietary patterns and the correlation between dietary inflammatory index and metabolic index were analyzed. Results: There was a high proportion of overweight,obesity and abdominal obesity among adult residents in Shanxi Province,and the prevalence of hypertension,hyperglycemia and metabolic syndrome was also high.By factor analysis,four dietary patterns were obtained:grain pattern,meat pattern,meat pattern and oil and salt pattern.The dietary inflammation index was positively correlated with waist circumference.Abdominal obesity was positively correlated with systolic blood pressure and blood glucose levels. Conclusion: High inflammatory dietary pattern may be associated with chronic metabolic disorders.Improving dietary patterns,especially increasing the intake of antioxidants and anti-inflammatory substances,may help reduce the risk of chronic inflammatory diseases.It is recommended to promote healthy dietary patterns through public health interventions to prevent and manage chronic diseases.

Longitudinal study on psychological resilience in patients with coronary heart disease after PCI

Objective: To explore the change trend and influencing factors of psychological resilience in patients with coronary heart disease after PCI. Methods: A longitudinal survey was conducted on 180 patients with coronary heart disease who were hospitalized and underwent PCI in a tertiary hospital in Nanning,Guangxi from October 2021 to October 2022 by using the general data questionnaire,10⁃iterm Connor⁃Davidson Resilience Scale,The Positive and Negative Affect Scale and the Medical Coping Modes Questionnaire. Results: The psychological resilience scores of patients with coronary heart disease after PCI during hospitalization,1 month after operation,3 months after operation and 6 months after operation were 23.77±4.61,27.02±4.04,26.60±3.65 and 27.61±3.62,respectively.The development trend of psychological resilience was affected by BMI,drinking history and number of diseased coronary arteries(<0.05).Positive emotions,negative emotions,and resignation coping had significant effects on the change trend of resilience.The effect of negative emotions on resilience decreased with time.The influence of positive affect,face coping,avoidance coping and yield coping on psychological resilience does not change with time. Conclusion: The psychological resilience of patients with coronary heart disease after PCI showed a slow upward trend within 6 months after operation.BMI index,drinking history and number of diseased coronary arteries affect the growth rate.Positive emotion,negative emotion and yield coping affect the change of resilience of patients.Positive emotion has a positive prediction on resilience,while negative emotion and yield coping have a negative prediction on resilience.The influence of negative emotions on psychological resilience will gradually decrease with time.
