Expert consensus on GD2 antibody dinutuximab beta for neuroblastoma care

Based on clinical research reports in China and abroad,as well as clinical practice experience of the participants in this study,relevant content on ganglioside⁃2(GD2) antibody dinutuximab beta for neuroblastoma care was thoroughly investigated through literature analysis,expert inquiries,and expert discussions.The final expert consensus on the use of GD2 antibody dinutuximab beta for neuroblastoma care was formed,which included preparation and requirements,drug configuration,drug infusion,care during infusion,and observation and care of adverse reactions.

Construction of classification model for oral medication in inpatient departments based on deep learning

Objective: To construct an oral medication classification model for inpatient pharmacies based on deep learning. Methods: Actual scenes were simulated,95 types of pill picture were collected to construct dataset,and pictures in dataset were preprocessed.Classification model for pills was constructed based on MobileNet V2 network,and Squeeze-and⁃excitation networks were embeded in the model to enhance the network's feature channel dependencies.Method of transfer learning was applied,using the pill dataset built by researchers to train and test the model,and the performance of the model was tested through the classification accuracy and the parameter quantity of the model. Results: The constructed model in this study demonstrates outstanding performance in classifying oral pill images collected in natural environments.Trained and tested on a self⁃built dataset comprising 95 categories and a total of 728 images,the accuracy of model classification was 95.8%.This outperforms MobileNet V2,ShuffleNet V2,and ResNet50 by 11.6%,14.3%,and 11.3%,respectively.The amount of model parameters was 2.55 M,which was approximately 1 out of 10 of ResNet50. Conclusions: The model constructed in this paper better balances the complexity and classification accuracy of the model,and provides a technical route and effect verification for the automatic pill classification system involved in pharmacies and other scenarios.It has certain theoretical and practical application value for improving the level of nursing automation in specific situations such as pharmacy dispensing and ward drug distribution.

Study on the effect of social participation on the health of widowed elderly people in China

Objective: To explore the effect of social participation on the health of widowed elderly people in China. Methods: A total of 6 296 samples were included based on the 2018 Chinese Longitudinal Healthy Longevity Survey,and the effect of social participation on the health of widowed elderly people in China was empirically analyzed by ordinary least squares and instrumental variables⁃two stage least squares. Results: The physical and mental health of widowed elderly people were improved by social participation.Endogeneity tests were conducted using "whether one owns property rights or not" and "whether the community organizes social activities and entertainment activities or not" as instrumental variables of social participation. The results showed that social participation still had a positive impact on the physical and mental health of widowed elderly people.After health indicators were replaced,and the conclusion had been proven to be robust. Conclusions: In actively responding to the aging population,widowed elderly people should be encouraged to participate in social activities,and detailed and differentiated plans should be developed based on characteristics of widowed elderly people,so as to create objective conditions for widowed elderly people to participate in social entertainment activities.

Study on temporal variation of hemodynamic indicators in the affected limb of patients undergoing total joint arthroplasty treated with venous foots pumps

Objective: To explore temporal variation of hemodynamic indicators in the affected limb of patients undergoing total joint arthroplasty(TJA) treated with venous foots pumps(VFP). Methods: A total of 30 patients who underwent TJA in the orthopedics department of a tertiary grade A hospital in Beijing were selected as the research subjects from January to March 2021.Hemodynamic indicators such as peak flow velocity(PFV),mean flow velocity(MV),blood flow(BF),and vessel diameter(Vd) of the femoral vein and popliteal vein in the affected limb of patients were collected by color Doppler ultrasound before applying VFP,after applying VFP for 10 minutes,after applying VFP for 30 minutes,after applying VFP for 1 hour,after stopping VFP for 10 minutes,and after stopping VFP for 30 minutes.Then,the temporal variation of hemodynamic indicators was analyzed. Results: The PFV and MV of patients after applying VFP for 30 minutes were faster than those before applying VFP(all <0.05). However,there was no statistically significant difference in PFV, MV,and BF of patients between after applying VFP for 1 hour and after applying VFP for 30 minutes(all >0.05).There was no statistically significant difference in PFV,MV,BF,Vd of patients between after stopping VFP for 10 minutes and after stopping VFP for 30 minutes(all >0.05). Conclusions: VFP can accelerate the venous blood flow velocity in the lower limbs of patients with TJA.It is recommended that single application of VFP should not be less than 30 minutes,and the intermittent duration should not exceed 10 minutes in clinical practice.

Construction of model of pediatric nurses' willingness to adopt local anesthetics in clinical practice

Objective: To explore the influencing factors of pediatric nurses' willingness to adopt local anesthetics in clinical practice. Methods: A total of 251 pediatric nurses from 10 hospitals in Sichuan province were surveyed by a self⁃designed survey questionnaire from November 1 to 15, 2022.Model of pediatric nurses' willingness to adopt local anesthetics in clinical practice was constructed based on innovative diffusion theory and theory of planned behavior.AMOS 24.0 software was used for structural equation modeling analysis. Results: The relative advantages and ease of use of local anesthetics in clinical application had a positive impact on the attitude of pediatric nurses.The attitude,subjective norms,and perceived behavioral control of local anesthetics in clinical application had a positive impact on willingness to adopt them.Model Chi⁃square degree of freedom ratio was 1.515, goodness of fit index was 0.910,adjust the goodness of fit index was 0.880,root mean square residual was 0.045, and root⁃mean⁃square residual was 0.020.This model could explain 71.4% variability of adoption willingness in pediatric nurses. Conclusions: The subjective norms,perceived behavioral control,and attitudes have positive impact on pediatric nurses' willingness to adopt local anesthetics in clinical practice.Among them,attitude has the strongest impact.The relative advantages and ease of use have a positive impact on pediatric nurses' willingness to adopt local anesthetics in clinical practice through attitude.This model has strong explanatory power for pediatric nurses' willingness to adopt local anesthetics to alleviate venous puncture pain in children.

Longitudinal study of symptom cluster in patients with esophageal carcinoma during concurrent chemoradiotherapy

Objective: To explore the dynamic changes of symptom groups at different time points in patients with esophageal carcinoma during concurrent chemoradiotherapy. Methods: A total of 102 patients with esophageal carcinoma treated in the oncology department of a tertiary grade A hospital in Sichuan province were selected as the research subjects from February to October 2022.General information questionnaire and MD Anderson Symptom Inventory for Gastrointestinal Cancer were used to survey patients before the start of chemoradiotherapy,at the time of 10 rounds of radiotherapy,at the time of 20 rounds of radiotherapy,and at the end of radiotherapy.Symptom groups were extracted by exploratory factor analysis. Results: During concurrent chemoradiotherapy,the incidence and severity of symptoms in patients with esophageal carcinoma were different at different time points,among which the incidence and severity of dysphagia,pain,fatigue and lethargy were higher.The exploratory factor analysis results showed that,there were 4,5,4 and 4 symptom groups were extracted before the start of chemoradiotherapy,at the time of 10 rounds of radiotherapy,at the time of 20 rounds of radiotherapy,and at the end of radiotherapy.Among them,psychological and physical discomfort symptom groups,digestive tract symptom group and eating difficulty symptom group were persistent,sleep disturbance symptom group existed before the start of chemoradiotherapy and the time of 10 rounds of radiotherapy.Adverse reaction symptom group began to develop from the time of 10 rounds of radiotherapy. Conclusions: There are multiple symptom groups in patients with esophageal carcinoma during concurrent chemoradiotherapy,and some symptom groups are persistence.The composition and severity of symptom groups are different at different time points.Medical staff should strengthen the symptom management of patients with esophageal carcinoma during concurrent chemoradiotherapy and improve the management plan to reduce the burden of symptoms of patients.

Construction of standardized program for enhanced recovery after surgery in patients with osteoporotic vertebral compressibility fracture

Objective: To construct standardized program for enhanced recovery after surgery in patients with osteoporotic vertebral compressibility fracture(OVCF). Methods: The literature was systematically reviewed.Initial draft of standardized program for enhanced recovery after surgery in OVCF patients was constructed based on evidence⁃based results.And final draft of standardized program for enhanced recovery after surgery in OVCF patients was constructed by expert inquiry method. Results: Two rounds of expert inquiry were conducted,with questionnaire response rate of 100%.The expert authority coefficients were 0.817 and 0.826,respectively.The Kendall's W were 0.250 and 0.337 (both <0.001).After second round of expert inquiry,the coefficient of variation of the indicators was all less than 0.200.Finally,standardized program for enhanced recovery after surgery in OVCF patients was constructed,which included 5 first⁃level indicators(health education and psychological intervention,medication intervention,postoperative pain and dietary management,postoperative rehabilitation training,and prevention of postoperative complications),11 secondary⁃level indicators,and 31 tertiary⁃level indicators. Conclusions: The standardized program for enhanced recovery after surgery in OVCF patients constructed is systematic,specific,the method is scientific,and the conclusion is reliable.

Analysis of influencing factors on postoperative kinesophobia in patients undergoing anterior cruciate ligament reconstruction

Objective: To explore influencing factors on postoperative kinesophobia in patients undergoing anterior cruciate ligament reconstruction(ACLR) and conduct pathway analysis. Methods: A total of 225 patients who planned to undergo ACLR in the sports medicine department of a tertiary grade A hospital in Shanxi province were selected by purposive sampling method as the research subjects from August 2021 to November 2022.General information questionnaires,Self⁃Perceived Burden Scale,Lysholm Knee Score,Pain Catastrophizing Scale,Connor⁃Davidson Resilience Scale and Tampa Scale for Kinesiophobia were used to investigate patients. Results: The score of Tampa Scale for Kinesiophobia in 255 postoperative patients undergoing ACLR was 43.25±9.89 points,and the incidence of kinesophobia was 65.9%.The timing of surgery,self-perceived burden,psychological resilience,and pain catastrophizing were the main influencing factors(<0.05) for the occurrence of postoperative kinesophobia in patients undergoing ACLR.The path analysis results showed that the pain catastrophizing and self-perceived burden not only had direct impact on the development of kinesophobia, but also had an indirect impact on the development of kinesophobia through psychological resilience. Conclusions: The incidence of kinesophobia in postoperative patients undergoing ACLR is high.Medical staff should pay attention to the psychological status of postoperative patients undergoing ACLR,adopt targeted intervention measures and health education programs to improve their psychological resilience and reduce the incidence of kinesophobia.

Construction of evaluation index system for enhanced recovery of patients after gynecological tumor surgery based on the three⁃dimensional quality structure model

Objective: To construct an evaluation index system for enhanced recovery of patients after gynecological tumor surgery. Methods: Evaluation index system for enhanced recovery of patients after gynecological tumor surgery was constructed through literature review and expert inquiry.And the weight of each index was determined by sampling were precedence chart method and analytic hierarchy process. Results: Two rounds of expert inquiry were conducted,with a questionnaire response rate of 100%,expert authority coefficients of 0.936 and 0.950,respectively.In the second round of expert inquiry,the Kendall harmony coefficient ranged from 0.556 to 0.764(all <0.001),the coefficient of variation for second⁃level indexes ranged from 0.000 to 0.178,and the coefficient of variation for third⁃level indexes ranged from 0.000 to 0.193.The final evaluation index system for enhanced recovery of patients after gynecological tumor surgery included 3 firs⁃level indexes,9 second⁃level indexes and 52 third⁃level indexes. Conclusions: The evaluation index system for enhanced recovery of patients after gynecological tumor surgery construct in this study is scientific and reliable,which can standardize the implementation of enhanced recovery after surgery,ensure patient safety,and provide scientific basis for the quality evaluation of perioperative enhanced recovery of patients after gynecological tumor surgery.
