Interpretation of 2023 "ESPEN practical guideline: home parenteral nutrition"

In January 2023, the European Society for Parenteral Nutrition released the latest home parenteral nutrition guidelines:" ESPEN practical guideline:home parenteral nutrition",aiming to provide appropriate and safe recommendations for clinical practice.The guideline builds on previously published guidance,provides the latest evidence and opinions,and converts recommendations and statements into a more logical flow chart format. The guideline includes 71 recommendations and 5 statements from 6 aspects,including indications for home parenteral nutrition, central venous access devices and infusion pumps,infusion catheters and central venous access devices nursing, nutritional mixed solution, and program monitoring and management.This article interprets the main content of the guideline,in order to provide a useful reference for the development and application of home parenteral nutrition in China.This paper interprets the main content of the guide, hoping to provide beneficial reference for the development and application of home parenteral nutrition.

Latent profile analysis of perceived stress among nurses in Anhui province and its influencing factors

Objective: To identify the latent profile of perceived stress among nurses in Anhui province and explore its influencing factors. Methods: A total of 33 330 nurses in Anhui province were selected from the platform of Anhui Provincial Nursing Association to conduct a questionnaire survey to evaluate the perceived stress, professional identity and psychological capital of nurses. Results: The perceived stress of nurses in Anhui province was divided into four categories: high demand and low stress group (34%,=11 401),stable medium stress group (48%,=16 179),low tension and high out of control group (8%,=2 582) and high tension and low out of control group (10%,=3 168).Gender,education level, professional title, monthly income,working years,number of children,professional identity and psychological capital were the influencing factors of the potential profile of perceived stress in nurses. Conclusion: The perceived stress of nurses in Anhui province can be divided into four potential categories: high demand and low stress group,stable medium stress group,low tension and high tension and low loss of control group.Identifying the potential categories is helpful to assess the risk of increased perceived stress of nurses, and carry out appropriate mental health education according to the characteristics of perceived stress of nurses,so as to reduce the stress level of nurses.

Translation and reliability and validity evaluation of Difficulties in End of Life Discussions⁃Family Inventory

Objective: To translate Difficulties in End of Life Discussion⁃Family Inventory (DEOLD⁃FI) into Chinese and test its reliability and validity among cancer patients. Methods: The DEOLD⁃FI was authorized and conducted according to the principle of scale introduction.The reliability validity of the scale was tested in 315 cancer patients. Results: Five public factors were extracted from Chinese version DEOLD⁃FI,which were emotional burden,negative attitude,family burden,fear of death, and communication ability,and the cumulative contribution rate of variance was 64.772%.The Cronbach's α coefficient of the total table was 0.897,the split⁃half reliability was 0.828,the test⁃retest reliability was 0.938,and the Cronbach's α coefficient of each dimension was above 0.85. Conclusion: The Chinese version of DEOLD⁃FI has good reliability and validity, and it can be used to evaluate the degree of end⁃of⁃life communication disorder with family members.

Cognition and experience of nursing staff in different regions regarding high⁃risk meteorological factors for stroke and their management strategies

Objective: To explore the meteorological-based influencing factors and coping strategies for the prevention and management of high-risk stroke populations through the cognition and experience of nursing staff in different regions on meteorological factors at high risk of stroke. Methods: From February to April 2023,15 nursing staff from tertiary hospitals in Lanzhou, Xi'an, Shenyang, Jinzhou, Changchun,Nanjing,Suzhou, Guangzhou, Dongguan were selected using purposive sampling. In-depth interviews were conducted using the thematic analysis method of qualitative research, and the Colaizzi 7⁃step method was used to analyze the interview data. Results: Two major themes were extracted: 1)cognition and experience of the impact of meteorological factors on stroke onset, including cognition of the impact of temperature, season, air pressure, and humidity on stroke onset; and 2) management strategies for stroke onset based on meteorological factors, including management strategies related to home life, family support, information support, health education, and medication guidance. Conclusion: Meteorological factors are closely related to stroke. Changes in meteorological factors can cause subtle changes in the body and induce stroke. To prevent the onset of stroke, attention should be paid to the relationship between stroke onset and meteorological factors and a health management plan for high-risk stroke populations based on meteorological factors should be developed.

Status quo and potential profile analysis of information competence of pediatric nurses in Hunan province

Objective: To explore the information competency of pediatric nurses in Hunan province, and analyze its potential profile and its influencing factors. Methods: From June 2023 to September 2023,458 pediatric nurses from a total of 4 tertiary grade A hospitals in Xiangdong, Xiangxi, Southern and Northern Hunan province were selected as the study objects by convenient sampling method General data questionnaire, Nurses Information Competency Assessment Scale, Nurses Using Mobile Nursing Information System Satisfaction Questionnaire,Clinical Nursing Information System Effectiveness Evaluation Scale were used to investigate. The potential profile of pediatric nurses' information competency was analyzed, and the influencing factors were identified by Logistic regression analysis. Results: The information competency score of pediatric nurses in Hunan province was (71.83±18.45), and their information competency was divided into three sections, namely low information competency group(31.4%),medium and low information competency group(57.2%) and high information competency group(11.4%).Logistic regression showed that age, education, years of work, position, title, night duty, mobile nursing information system satisfaction and clinical nursing information system effectiveness were the factors influencing the potential profile of pediatric nurses' information competency(<0.05). Conclusion: The information competence of pediatric nurses in Hunan province should be enhaned, and there is heterogeneity. Schools and hospitals should pay attention to the importance of information competence in nurses' career, and conduct individualized intervention for pediatric nurses according to different potential profile factors, so as to improve the overall quality of pediatric nursing.

Influencing factors of dysfunction in patients with COPD based on random forest algorithm

Objective: To explore the current status of functional impairment and its influencing factors in patients with COPD, and to provide a reference basis for the prevention and control of COPD. Methods: A total of 214 patients with COPD in three tertiary care hospitals in Tianjin from June 2020 to March 2021 were selected and questionnaires were administered using a general information questionnaire, the modified version of the dyspnoea questionnaire (mMRC), the COPD assessment scale (CAT) and the brief version of the ICF⁃COPD core set.And the importance of influencing factors was assessed by the random forest algorithm.The significant influencing factors were ranked, screened for multiple covariance,and logistic regression models were applied to explore their influencing factors. Results: COPD patients had a total functional impairment score of (15.13±8.36) and the random forest algorithm showed that the maximum number of features to generate the best forest was six.The top six influencing factors for analysis were CAT score, presence of acute exacerbations in the last year,personal substance use,environment, GOLD classification and presence of comorbidities. The random forest algorithm was used to screen the variables for Logistic regression, and the results showed that the COPD assessment test (OR=11.091),e110 "supplies or substances for personal consumption" (OR=5.136), presence of comorbidities (OR=2.480), presence of acute exacerbations in the last year (OR=2.347) were influential factors for functional impairment in COPD patients. Conclusion: More severe disease symptoms,impaired access to or effective use of medication, having comorbidities, a history of acute exacerbations in the last year and a higher GOLD classification were the five factors that had a greater impact on functional impairment in patients with COPD. Clinical caregivers should pay attention to the functional impairment of patients with COPD and target early identification and intervention.

Drug knowledge discovery for autism spectrum disorders based on SPO predications

Objective: To extract SPO(Subject⁃Predicate⁃Object,SPO) from literature related to Autism Spectrum Disorders(ASD) using semantic mining technology and construct a knowledge graph of ASD drug entities,to explore the potential drug for the treatment of ASD at a deeper level, and provide new ideas for discovering valuable potential drugs for other diseases(). Methods: Using the tools SemRep and Metamap based on the Unified Medical Language System (ULMS) to process ASD literature records and obtain SPO of ASD drug entities.The Neo4j database was used for knowledge storage to construct an ASD drug entities knowledge graph.Using three semantic pathways to discovery ASD drug knowledge based on the knowledge graph.Then verified and analyzed the effectiveness of the results in the clinical trials databases. Results: The SPO obtained includes 1 262 head entities, 687 tail entities, and 18 entity relationships.A total of 32 drugs were discovered through three semantic pathways,27 potential drugs for ASD was screened out,and 19 drugs can be validated in the clinical trials databases. Conclusions: The knowledge discovery of ASD drugs based on knowledge graph which built by SPO can provide a certain theoretical and methodological basis for drug repositioning,provide new ideas for traditional drug discovery,and provide decision support for clinical experiments and scientific research.

Dynamic of perioperative anxiety in patients undergoing femtosecond laser small incision lenticule extraction and its influencing factors: a longitudinal study

Objective: To explore the dynamic change trend and influencing factors of perioperative anxiety in SMILE patients at different times. Method: A total of 226 patients in the optometry center of a hospital in Zhengzhou were selected as the study subjects. General information questionnaire, Psychological Resilience Scale, Medical Coping Style Questionnaire,Trait⁃Anxiety Scale were used to investigate factors before operation and State-Anxiety Scale was used to investigate anxiety before operation(T0),on the day of operation(T1), one day after operation(T2), one week after operation(T3) and one month after operation(T4).The scores of anxiety were analyzed by repeated measurement analysis of variance to explore the temporal effects of anxiety,correlation analysis to explore the relevant factors of each anxiety and generalized estimation equation to explore the influencing factors of perioperative period. Results: 208 patients were enrolled.The anxiety levels of SMILE patients at five time points during the perioperative period were (34.25±8.77),(38.22±9.25),(36.89±11.28), (34.04±11.02) and (30.50±9.91) respectively.The results of correlation analysis showed that the degree of myopia, the relationship with family, the yielding face style, resilience, and trait anxiety were all correlated with anxiety at five time points, while the gender was correlated with anxiety at T0, T1, T2, and T4, while the living environment was only correlated with anxiety at T4. The results of the generalized estimation equation showed that the influencing factors of perioperative anxiety included gender,monthly income,age,educational level, living environment, surgical history, resilience, yielding coping style and trait anxiety. Conclusion: Perioperative anxiety of SMILE patients increased first and then decreased, and the influencing factors were different at different time points. Therefore, nurses could formulate targeted nursing measures according to different characteristics of patients at different time points to reduce anxiety, which also provides a reference for reducing the anxiety of other surgical patients in the outpatient department.

Influencing factors of fear of progress in maintenance hemodialysis patients and their spouses based on random forest model

Objective: To explore the influencing factors of fear of progression in middle⁃aged and elderly maintenance hemodialysis patients and their spouses by using random forest algorithm. Methods: Using convenient sampling method, 239 middle-aged and elderly maintenance hemodialysis patients and their spouses who were treated in hemodialysis centers of tertiary grade A hospitals in Luzhou were investigated, and the importance of influencing factors was ranked by random forest model. Results: The total score of fear of progression in middle⁃aged and elderly FoP patients was (38.48±8.49), and the total score of fear of progression in spouse was (38.05±8.21). Random forest importance ranking shows that dialysis age, positive coping style and age of patients were the top three important factors affecting the fear of progression in middle⁃aged and elderly patients. Spouse's positive coping style, spouse's illness perception and spouse's social support are the top three important factors affecting the progress of spouse's fear of progression in middle⁃aged and elderly maintenance hemodialysis patients. Conclusion: Middle⁃aged and elderly maintenance hemodialysis patients and their spouses generally have serious fear of progression. Clinical medical staff should pay attention to it and formulate personalized intervention measures for patients and their spouses with different characteristics.

Development and reliability and validity of exercise benefits and barriers perception scale for hip/knee throplasty patients

Objective: To develop and test the reliability and validity of the exercise benefits and barriers perception scale for hip/knee arthroplasty patients. Methods: Through literature review,semi⁃structured interview,expert inquiry and pre⁃investigation, the exercise benefits and barriers perception scale for hip/knee arthroplasty patients was formed,and 243 and 204 patients with hip/knee arthroplasty were investigated independently twice. Results: Five common factors were extracted by exploratory factor analysis,which were life and health benefits,functional rehabilitation benefits, disorder of disease symptoms, disorder of adverse consequences, and disorder of exercise information,and the cumulative variance contribution rate was 75.235%.The confirmatory factor analysis showed that the model fit well,and was significantly correlated with the Exercise Benefits/Barriers Scale (=0.617) and the Frenchay Activities Index (=0.584).The total Cronbach's αof final scale was 0.829,the half⁃reliability was 0.722, and the retest reliability was 0.861 after 3 weeks. Conclusion: The exercise benefits and barriers perception scale for hip/knee arthroplasty patients has good reliability and validity,and it can be used to evaluate the exercise benefit and barriers perception of hip/knee arthroplasty patients.

Construction of a Nomogram model for risk of PICC related upper extremity DVT in elderly patients

Objective: To construct a Nomogram predicting model for assessing the risk factors of peripherally inserted central catheter(PICC) related upper extremity deep venous thrombosis(DVT)in elderly patients, and to evaluate the discrimination and consistency of the model. Methods: The clinical data of 226 elderly patients with PICC in researched hospital from January 2014 to December 2019 were retrospectively analyzed by self⁃design questionnaire.The incidence of catheter⁃related upper extremity DVT was calculated.Research subjects were divided into DVT group and non DVT group.The risk factors related to upper extremity DVT occurrence were screened by Logistic regression model,and the risk factors were included in R software to construct the risk factors of upper extremity DVT.The discrimination and consistency of the prediction model were evaluated by ROC curve and goodness⁃of⁃fit test. Results: The incidence of upper extremity DVT in elderly patients with PICC was 20.35%(46/226).The results of Logistic regression model showed that the age,malignant tumor,D⁃D level before catheterization and operation history during catheterization were independent risk factors for upper extremity DVT in elderly patients with PICC(<0.05);The area under the ROC curve of the Nomogram model model is 0.777,the slope of calibration curve was close to 1,and the Hosmer⁃Lemeshow goodness⁃of⁃fit test showed =9.637,=0.291. Conclusion: The Nomogram model based on the risk factors of PICC related upper extremity DVT in elderly patients has good discrimination and consistency,which has certain clinical guiding value for the prevention of upper extremity DVT in elderly patients with PICC.
