Quality evaluation of ostomy care guidelines/consensus and content analysis of peripheral skin care products

Objective: To evaluate the methodological quality of ostomy nursing guidelines,expert consensus and group standards,and analyze the relevant recommendations of skin protection products around ostomy,so as to provide evidence-based basis for the development of comprehensive ostomy nursing guidelines in China. Methods: Guidelines,expert consensus and group standards related to ostomy care were systematically and comprehensively searched in major Chinese and English databases,guideline databases,and websites of guideline societies/associations until December 2022.The quality evaluation tools of guidelines and consensus were used to evaluate the quality of the included literature,and the recommendations related to skin care products around the stoma were extracted and compared. Results: A total of 8 guidelines were included,all of which were foreign guidelines.3 expert consensus,2 from abroad,1 from China;there is only one group standard in China.In the eight guidelines,three can be rated A and are of high quality,while the quality of expert consensus and group standards is generally low.All included guidelines,consensus,and group standards recommend the use of skin protection products for the care of peristomal skin,but most recommendations do not indicate specific selection and use. Conclusions: The overall methodological quality of ostomy care guidelines,expert consensus,and group standards needs to be improved.The recommendations related to skin protection products are not specific,comprehensive,and the evidence base is insufficient.It is expected that domestic researchers can develop comprehensive evidence-based guidelines for localized ostomy care in the later stage,so as to correctly guide the clinical rational selection and use of skin care products around the stoma.

Development of a decision aid to support decision making in laser therapy for patients with primary angle closure

Objective: Develop a decision aid for laser therapy of patients with primary angle closure based on the shared decision-making concept to provide professional decision support for patients,improve the quality of decision-making and promote shared decision⁃making. Methods: Based on the Ottawa Decision Support Framework and International Patient Decision Aids Standard,the first draft of the laser therapy decision aid for patients with primary angle closure was developed through literature analysis.Through two rounds of expert consultation,integrating input from experts in the fields of glaucoma clinical medicine,ophthalmic nursing and public health management,resulting in a revised version.Clinical investigation was conducted to understand the evaluation of the acceptability of the tool by patients and medical staffs,and the tool was further refined to form the final draft. Results: A total of 19 experts completed this expert consultation with a positivity of 95.0% and an authority factor of 0.918.After two rounds of expert consultation,the Kendall's W for each indicator of the tool were 0.131,0.224 and 0.187 respectively(<0.05),indicating that the experts were more accepting of the content of the tool and had a good degree of harmonization of opinions.Clinical investigation found a high level of acceptance of the tool by patients and medical staffs,who found it easy to understand and use,and able to assist in decision-making.The final laser therapy decision aid for patients with primary angle closure contains 7 primary indicators:clarify decision,assess decision needs,provide information,analyse risk-benefit,clarify values,guide decision making and confirm decision,15 secondary indicators and 32 tertiary indicators. Conclusion: The laser therapy decision aid for patients with primary angle closure is scientifically and clinically applicable,and provides a scientific and effective means of supporting patient participation in their own safety management and achieving shared decision⁃making.

Constructing a joint training program for core competencies of nursing degree postgraduates and oncology nurses based on Delphi method

Objective: To construct a joint training program for core competencies of nursing degree postgraduates and oncology nurses,and to provide reference for promoting the development of nursing degree postgraduate education in China. Methods: Through literature analysis and semi⁃structured interviews,we initially formed a pool of entries for the joint cultivation program of core competencies between nursing graduate students and oncology nurses,developed an expert consultation questionnaire,and conducted expert consultation with 15 experts by Delphi method. Results: The positivity coefficient of the selected experts in the three rounds of consultation was 100.0%,and the authority degree of the experts was 0.838,0.880 and 0.880 respectively.5 primary indicators,17 secondary indicators and 96 tertiary indicators,including theoretical knowledge mastery ability,clinical practice application ability,organization and management ability,teaching and research ability and development ability,were finally formed after the consultation.The joint training program of core competencies for nurses in specialties. Conclusion: The joint cultivation program of core competencies of nursing graduate students and oncology nurses established in this study has good reliability and scientificity,and provides some reference value for the cultivation of specialized talents in oncology nursing.

Construction of nursing evaluation indicators of specialized medical telecare for patients with non⁃valvular atrial fibrillation after interventional surgery

Objective: To construct the nursing evaluation indicators for patients with non⁃valvular atrial fibrillation after interventional surgery,aim to provide reference for remote care in China. Methods: Common nursing issues for patients with non⁃valvular atrial fibrillation after interventional surgery were integrated through literature review and analysis,based on the Omaha problem classification system,relevant nursing issues were sorted and classified,Delphi expert consultation method was used to conduct two consecutive rounds of expert on the included indicators,than constructed the nursing evaluation indicators for patients with non⁃valvular atrial fibrillation after interventional surgery. Results: The recovery rate of the 2 rounds of questionnaires was 100%,the authority coefficient(Cr) of experts was 0.881 and 0.910,and Kendall's W value was 0.711 and 0.821,respectively(<0.05).The nursing evaluation indicators for patients with non valvular atrial fibrillation after interventional surgery included 5 first⁃level indicators,16 second⁃level indicators and 62 third⁃level indicators,the coefficient of variation(CV) of each item was<0.25. Conclusion: The nursing evaluation indicators for patients with non⁃valvular atrial fibrillation after interventional surgery constructed is scientific and reliable,not only ensures the feasibility of clinical implementation,but also provides reference for remote care in specialized medical care in China.

Construction of perioperative nursing sensitive index system for mediastinal tumor resection under video⁃assisted thoracoscope

Objective: To construct a perioperative nursing sensitive index system for mediastinal tumor resection under video⁃assisted thoracoscope,aiming to provide reference for promoting the quality of specialized nursing care in hospitals. Methods: The quality standard system of perioperative nursing care for mediastinal tumor resection under video⁃assisted thoracoscope was initially formulated through systematic literature review,semi⁃structured interview and hospital expert meeting based on the three⁃dimensional quality evaluation model of “structure⁃process⁃result”,Delphi method was used for inquiry,demonstration and modification,and the weight was determined by analytic hierarchy process. Results: The questionnaires recovery rates of the two rounds of expert correspondence were 85.71% and 100.00% respectively,the expert authority coefficient(Cr) of the second round expert correspondence was 0.897;The importance assignment score of each index in the second round of correspondence were 4.23⁃5.00,and the coefficient of variation(CV) was 0.000⁃0.147;The Kendall'sW values of the two rounds of correspondence were 0.714 and 0.825 respectively(<0.05).The constructed perioperative nursing sensitive index system for mediastinal tumor resection under video⁃assisted thoracoscope included 3 first⁃level indicators,8 second⁃level indicators and 46 third⁃level indicators. Conclusion: The constructed perioperative nursing sensitive index system for mediastinal tumor resection under video⁃assisted thoracoscope is scientific and reliable,the selected evaluation index can comprehensively,accurately and systematically provide a basis for the continuous improvement of nursing quality in thoracic surgery specialty.

Construction of Nomogram prediction model for cardiac adverse events in bedridden constipation patients after PCI surgery

Objective: To analyze the risk factors of adverse cardiac events in bedridden constipation patients after PCI and establish a predictive nomogram model. Methods: A total of 248 patients with constipation in bed during PCI perioperative period admitted to our hospital from November 2020 to October 2021 were selected as the research objects.According to whether the patients had cardiac adverse events during hospitalization,they were divided into cardiac adverse events group and non-cardiac adverse events Group.Collect two groups of clinical data for single factor and binary logistic regression analysis to obtain independent predictive factors,build a nomogram risk model based on this,and evaluate the predictive efficacy of the model. Results: The incidence of adverse cardiac events was 17.3%.Univariate and binary logistic regression analysis showed that patients were ≥68 years old (OR=9.970,95%CI 2.599⁃38.251),defecation in supine position (OR=3.308,95%CI 1.074⁃10.188),D dimerization body ≥402.99 ng/mL(OR=29.901,95%CI 8.800⁃101.601) and left ventricular ejection fraction≤47.46%(OR=23.540,95%CI 7.118⁃77.853) are patients with bedridden constipation after PCI with heart Independent risk factors for adverse events(<0.05).A nomogram risk model was established based on the above influencing factors.The model verification results showed that the model consistency index(C⁃index) was 0.955,and the calibration curve showed that the nomogram model had good discrimination and consistency. Conclusions: The age,defecation position,D⁃dimer level,and left ventricular ejection fraction of patients with constipation in bed after PCI are closely related to the occurrence of adverse cardiac events.The nomogram model established based on this is helpful for clinical nurses to identify high⁃risk early patient.

Construction and validation of a predictive model for postpartum fever risk in women undergoing epidural⁃related maternal fever

Objective: To construct a risk prediction model of epidural⁃related maternal fever(ERMF) and verify the effect. Methods: A prospective study was conducted in a Grade-A obstetrics and gynecology hospital in Jiangsu Province.A total of 780 parturients who received epidural analgesia in the delivery room from January 2022 to July 2022 were selected as subjects.According to diagnosis,they were divided into ERMF group(=148) and non⁃ERMF group(=632).The independent risk factors of ERMF were determined by Logistic regression analysis,and the prediction formula was obtained to construct the risk prediction model.Receiver operating characteristic(ROC) and Hosmer⁃Lemeshow test were used to evaluate the prediction efficiency and goodness of fit of the model.A total of 170 parturients hospitalized from August 2022 to October 2022 were selected as the validation group of the model to verify the prediction model.Results The incidence of ERMF was 19.0% in the modeling group and 20.6% in the validation group.Number of examinations of the vaginal tract,oxytocin use,labor,the time from rupture of membranes to the end of labor,ropivacaine dosage,sufentanil dosage was an independent risk factor of ERMF.The ROC curve of the prediction model was 0.987,the sensitivity was 0.912,the specificity was 0.796,and the Youden index was 0.888.The ROC curve of the verification group was 0.893,the sensitivity was 0.886,the specificity was 0.785. Conclusion: By verifying that the prediction model has good differentiation and calibration degree,it has certain prediction value for identifying high⁃risk ERMF parities,and can provide reference for clinical nursing work,so as to formulate corresponding nursing measures to reduce the risk of ERMF.
