Study on contents of prescribing right of nurses in department of gynecology and obstetrics

Objective: To discuss the specific prescription drugs and prescription forms of gynecology and obstetrics drugs that can be issued by gynecology and obstetrics nurses who conform to the prescribing power applicant qualification in tertiary grade A hospitals of China. Methods: The drugs that could be prescribed by gynecology and obstetrics nurses were preliminarily established through literature analysis and expert semi-structured interview.Medical and nursing experts were consulted by expert consultation for 2 rounds to determine the contents of prescribing rights and prescription forms of gynecology and obstetrics nurses. Results: The questionnaire recovery rates of 2 rounds of expert consultation were 75.00% and 88.24%,respectively.And authority coefficient of 2 rounds of expert consultation were 0.77 and 0.85,respectively.Finally,it was determined that there were 2 major categories,with 9 kinds of drugs can be issued by gynecology and obstetrics nurses in tertiary grade A hospitals of China,among which 1 kind of drugs tended to be prescribed in the form of protocol or extended prescription,6 kinds of drugs tended to be prescribed in the form of independent or protocol prescription,2 kinds of drugs tended to be prescribed in the form of independent prescription. Conclusions: Gynecology and obstetrics nurses who conform to the prescribing power applicant qualification can prescribe 9 kinds of gynecology and obstetrics drugs,mainly in the form of independent or protocol prescription.

Regularities of grouping of medicinal drink recipes in "Chinese medicinal diet dictionary"

Objective: To analyze the regularities of grouping of 3 kind of medicinal drink recipes in “Chinese medicinal diet dictionary”. Methods: "Chinese medicinal diet dictionary" was used as the prescription source.Traditional Chinese Medicine Inheritance Assistant Platform Software(V 2.5) was used for data analysis.The database included medicinal drink recipes with boiling type,soaking type,and brewing type was constructed.Data mining methods such as frequency statistics,association rules,and cluster analysis were used to analyze and compare the regularities of grouping of medicinal drink recipes. Results: A total of 861 medicinal drink recipes were included,including 570 medicinal drink recipes with boiling type, 168 medicinal drink recipes with soaking type, and 123 medicinal drink recipes with brewing type,involving raw materials were 442 types,207 types and 185 types,respectively.Association rule analysis showed that common raw materials combinations of the 3 kinds of medicinal drink recipes were 19,19 and 4,respectively.Cluster analysis showed that showed that new grouping of the 3 kinds of medicinal drink recipes were 15,5 and 8,respectively. Conclusions: The medicinal drink recipes with boiling type focuses on clearing heat,the medicinal drink recipes with soaking type focuses on dispersing wind-heat,and the medicinal drink recipes with brewing type focuses on tonic benefits.The medicinal drink recipes with boiling type mostly combine clearing heat and dispelling dampness,and the the medicinal drink recipes with soaking type focus on pungent and cooling relief of symptoms.The medicinal drink recipes with boiling type is preferred for clearing heat,the medicinal drink recipes with soaking type for pungent and cooling relief of symptoms,and the the medicinal drink recipes with brewing type for tonic benefits.

Pathway analysis of influencing factors on subjective cognitive decline in the elderly

Objective: To investigate the statue quo of subjective cognitive decline(SCD) in the elderly,and to analyze its influencing factors and pathways. Methods: A total of 1 004 elderly from 8 communities or townships in Zhejiang province were selected by two⁃stage cluster sampling method as the research subjects from July to November 2022.General information survey questionnaire,The 21⁃Item Subjective Cognitive Decline Questionnaire(SCD⁃Q21), Short⁃Form Mini⁃Nutritional Assessment(MNA⁃SF),15⁃Item Geriatric Depression Scale(GDS⁃15),3⁃Item Subscale of the Geriatric Depression Scale(GDS⁃3A),and International Physical Activity Questionnaire Short Form(IPAQ⁃SF) were used to investigate the elderly.And the influencing factors and pathways of SCD in the elderly were analyzed. Results: The total score of SCD⁃Q21 in elderly was 7.50(3.50,11.50).Hearing status,viewing the visual condition of distant objects,visual condition of objects in close proximity,number of medications used,education level,pain level,indifference,depression,hypertension,and decreased grip strength or not were the influencing factors of SCD in the elderly(<0.05),among them,indifference(0.245),hearing status(-0.184)and,visual condition of objects in close proximity(-0.138) were highly influential factor, indifference and depression were mediating factors for SCD in the elderly. Conclusions: The subjective cognitive level of the elderly needs further improvement,which is influenced by multiple factors and pathways.Indifference and depression have mediating effect on SCD.

The relationship between social participation,self⁃management,social cohesion and health among elderly people in central and western regions of China

Objective: To explore the relationship between social participation,self-management,social cohesion and health among elderly people in central and western parts of China. Methods: Based on the data of China General Social Survey in 2017,the Ordinal Logit nested model and KHB decomposition method were used to examine the effects and mechanisms of informal participation and formal participation on health among elderly people in central and western parts of China. Results: Informal participation and formal participation had significant positive effects on health among elderly people in central and western parts of China,and the effect was more obvious in male and married elderly people.Self-management and social cohesion were mediating variables of mechanism of social participation,but their mechanisms were different in dimension and structure.The mediating effect of self⁃management was significant in men,while the mediating effect of social cohesion was significant in women. The mediating effect of self⁃management was significant in both married and unmarried groups,while the mediating effect of social cohesion was significant in married groups. Conclusions: Policies should be implemented and government departments and medical care force should be mobilized to guide the social participation of the elderly people in central and western parts of China,so as to improve their health level.

Construction of risk prediction model for frailty and pre⁃frailty in patients with maintenance hemodialysis

Objective: To analyze the influencing factors of frailty and pre-frailty in patients with maintenance hemodialysis(MHD),and to construct risk prediction model for frailty and pre⁃frailty in MHD patients. Methods: A total of 260 patients who underwent MHD treatment in nephrology department of Deyang People's Hospital from July to October 2022 were selected as the research subjects.The general information questionnaire,Fried Frailty Phenotype,Sarcopenia-Five Scale(SARC⁃F),Geriatric Depression Scale 15(GDS⁃15), Mini⁃Mental State Examination(MMSE),Time Up and Go(TUG) and Five⁃Times⁃Sit⁃to⁃Stand Test(FTSST) were used to evaluate patients.Univariate and multivariate analysis were used to analyse influencing factors of frailty and pre⁃frailty in MHD patients.Risk prediction model for frailty and pre⁃frailty in MHD patients was constructed. Results: Among 260 MHD patients,there were 62 frail patients(23.85%) and 144 pre-frail patients (55.38%).Multivariate analysis showed that sedentary behavior,TUG test time,FTSST time,and SARC⁃F score were the influencing factors of frailty and pre⁃frailty in MHD patients(<0.05).The area under the curve of receiver operator characteristic of the risk prediction model based on the influencing factors of frailty and pre⁃frailty in MHD patients was 0.819,the maximum Youden index was 0.521,sensitivity was 74.3%,specificity was 77.8%,and diagnostic value was 1.272.The results of the Hosmer⁃Lemeshow goodness of fit test showed that =2.734,=0.950. Conclusions: The constructed risk prediction model for frailty and pre-frailty in MHD patients has good predictive performance.

Best evidence summary for management of inter⁃hospital transport in patients undergoing extracorporeal membrane oxygenation

Objective: To evaluate and summarize the best evidence for management of inter-hospital transport in patients undergoing extracorporeal membrane oxygenation(ECMO). Methods: The relevant literature regarding the management of inter⁃hospital transport in patients undergoing ECMO was retrieved in Chinese and foreign databases and related official websites.Literature quality was evaluated according to literature evaluation standards and evidence was extracted. Results: A total of 15 literature was included,involving 4 guidelines,3 systematic reviews,3 cohort studies,1 cross⁃sectional study,and 4 expert consensuses.A total of 28 pieces of best evidence for management of inter⁃hospital transport in patients undergoing ECMO were summarized,including 6 aspects with transport decision⁃making,transfer team construction,equipment consumables management,transfer implementation,transfer adverse events and quality control and training. Conclusions: Medical staff converted evidence into standardized management plan of inter⁃hospital transport in patients undergoing ECMO according to actual situation,which is beneficial for improving quality of transportation and ensuring patient safety.

Influencing factors of medication adherence among older patients with chronic heart failure:an analysis based on COM⁃B model

Objective: To investigate the status quo of medication adherence among older patients with chronic heart failure,and to analyze pathways of medication literacy,cognitive function,social support,and medication beliefs on medication adherence based on capacity,opportunity,motivation⁃behavior (COM⁃B) model. Methods: A total of 412 older patients with chronic heart failure admitted to the Department of Cardiology,Shanghai Fifth People's Hospital,Fudan University were conveniently selected as subjects from January to September 2022.General information questionnaire,Morisky Medication Adherence Scale(MMAS),Mini⁃Mental State Examination(MMSE),Social Support Rated Scale(SSRS),Medication Literacy Assessment in Spanish and English(MedLitRxSE) and The Beliefs about Medicines Questionnaire⁃Specific(BMQ⁃specific) were used for investigation.A structural equation modeling was adopted for path analysis. Results: The MMAS score of older patients with chronic heart failure was 5.75±1.75,189 patients(45.9%) had poor medication adherence.Medication literacy,cognitive function,social support,and medication beliefs had direct impact on medication adherence,and the effect were 0.278,0.099,0.127 and 0.531,respectively.Medication literacy also indirectly affected medication adherence through medication belief,with effect of 0.146.Cognitive function also indirectly affected medication adherence through medication literacy and medication beliefs,with effect of 0.167. Social support also indirectly affected medication adherence through medication literacy,with effect of 0.151. Conclusions: Older patients with chronic heart failure have low medication adherence level.Medication literacy,cognitive function,and social support all affect medication adherence through direct or indirect effects,and medication beliefs directly affect medication adherence.

Construction of oral nutritional supplements compliance assessment scale for patients with colorectal cancer

Objective: To construct oral nutritional supplements(ONS) compliance assessment scale for patients with colorectal cancer. Methods: The theory of knowledge,attitude,practice was used as the structural framework of scale,relevant guidelines and literature were used as reference materials,and a pool of items of ONS compliance assessment scale for patients with colorectal cancer was formed.Then,ONS compliance assessment scale for patients with colorectal cancer was constructed by expert inquiry method. Results: Two rounds of expert inquiry were conducted, with questionnaire response rates of 90.91% and 100.00%,expert opinion submission rates of 45.00% and 25.00%, and expert authority coefficients of 0.878 and 0.893,respectively.In the second round of expert inquiry,the average score of importance of the dimensions were from 4.80 to 4.90,the full score ratio were from 85% to 90%,the coefficient of variation were from 0.06 to 0.11,and the Kendall harmony coefficient was 0.874 (<0.001).The average score of importance of the items were from 4.30 to 4.95,the full score ratio were from 35% to 95%,the coefficient of variation were from 0.05 to 0.24,and the Kendall harmony coefficient was 0.410(<0.001).Finally,ONS compliance assessment scale for patients with colorectal cancer was constructed,which included 3 dimensions and 33 items. Conclusions: The constructed ONS compliance assessment scale for patients with colorectal cancer has good scientific and reliable validity,and can be used as a tool for evaluating ONS compliance in patients with colorectal cancer.

Intervention effect of energy conservation on cancer⁃related fatigue in patients with cervical cancer undergoing concurrent chemoradiotherapy

Objective: To explore the intervention effect of energy conservation on cancer-related fatigue(CRF) in patients with cervical cancer undergoing concurrent chemoradiotherapy. Methods: 108 patients with cervical cancer who were hospitalized in radiotherapy ward of the First Affiliated Hospital of Xi'an Jiao Tong University were selected as subjects by convenient sampling method: from April to December 2018.They were divided into control group and intervention group based on their admission time, with 54 patients in each group.The control group received routine nursing care in radiotherapy department,while the intervention group received intervention with energy conservation on the basis of routine nursing care in radiotherapy department.The Chinese version of The Revised Piper Fatigue Scale(RPFS) was used to evaluate severity of CRF in patients before treatment,after 5 rounds of radiotherapy,after 10 rounds of radiotherapy,after 15 rounds of radiotherapy, after 20 rounds of radiotherapy,and after 25 rounds of radiotherapy. Results: During the intervention period,there were 3 patients lost in control group and 4 patients lost in intervention group.Finally,51 patients in control group completed the study,and 50 patients in intervention group completed the study.The intervention main effect and time main effect of total score of the Chinese version of RPFS and its 4 dimensional scores with behavioral fatigue,emotional fatigue,perceived fatigue,and cognitive fatigue had statistically significant(<0.05).The total score of the Chinese version of RPFS and its each dimensional scores between control group and intervention group had not statistically significant difference(>0.05) at two time points that before treatment and after 5 rounds of radiotherapy.The total score of the Chinese version of RPFS and its each dimensional scores between control group and intervention group had statistically significant difference(<0.05) at four time points that after 10 rounds of radiotherapy,after 15 rounds of radiotherapy,after 20 rounds of radiotherapy,and after 25 rounds of radiotherapy.The median scores of total score of the Chinese version of RPFS and its each dimensional scores in control group were higher than or egual to those in intervention group. Conclusions: The level of CRF in patients with cervical cancer undergoing concurrent chemoradiotherapy increases during treatment period.The energy conservation can effectively reduce level of CRF in patients with cervical cancer undergoing concurrent chemoradiotherapy at each time points.

Influencing factors and prediction model construction of cognitive impairment in young⁃old patients with Parkinson's disease

Objective: To investigate the risk factors for cognitive impairment in the young⁃old patients with Parkinson's disease(PD),and to construct a prediction model based on the risk factors. Methods: A total of 164 young⁃old PD patients who completed the 5⁃year follow⁃up from PPMI database were included in this study.Montreal Cognitive Assessment(MoCA) was used to assess cognitive function. 164 young⁃old PD patients were divided into cognitive normal group(PD⁃NC) and cognitive impairment group(PD⁃CI).Logistic regression analysis was used to explore the risk factors for cognitive impairment in young⁃old patients with PD,and a predictive model was constructed based on these factors. Results: Among 164 young⁃old PD patients,101 were in the PD⁃NC group and 63 were in the PD⁃CI group, and the incidence of cognitive impairment was 38.4%.Logistic regression analysis showed that age,years of education,MDS⁃UPDRSⅡ and MDS⁃UPDRS Ⅲ scores entered the regression equation.The AUC of the constructed prediction model was 0.815. Conclusions: The cognitive function of young⁃old patients with PD is affected by age,years of education, activities of daily living and motor function.The established prediction model has good discrimination and calibration,which can provide a reference for early screening and intervention of cognitive impairment in young⁃old patients with PD.

Construction of nutrition management program for head and neck caner patients with patients complicated with radioactive oral cavity mucositis

Objective: To construct nutrition management program for head and neck caner patients with patients complicated with radioactive oral cavity mucositis. Methods: The guidelines,expert consensus,systematic evaluation and other literature on nutritional management for head and neck caner patients with patients complicated with radioactive oral cavity mucositis was retrieved,and the evidence was extracted and integrated.After discussion by research group,the first draft of nutrition management program for head and neck caner patients with patients complicated with radioactive oral cavity mucositis was constructed.Finally,the nutrition management program for head and neck caner patients with patients complicated with radioactive oral cavity mucositis was constructed. Results: The nutrition management program for head and neck caner patients with patients complicated with radioactive oral cavity mucositis included 5 first⁃level items(nutrition screening and evaluation,nutritional supply,nutritional support,nutritional follow-up,exercise intervention),17 second⁃level items and 40 third⁃level items,of which 28 third⁃level items were A⁃level recommendations and 12 third⁃level items were B⁃level recommendations. Conclusions: The nutrition management program for head and neck caner patients with patients complicated with radioactive oral cavity mucositis constructed in this study is scientific and targeted,and can provide reference for future nutrition management practice of head and neck caner patients with patients complicated with radioactive oral cavity mucositis.

Status quo and influencing factors of frailty in elderly diabetic patients

Objective: To understand status quo of frailty in elderly diabetic patients and analyze its influencing factors. Methods: A total of 260 elderly diabetic patients from the endocrinology department and wound repair department of a tertiary grade A hospital in Weihai city were selected as research subjects by convenient sampling method.And general information questionnaire,the Chinese version of Tilburg Frailty Indicator(TFI),the Chinese version of traditional Chinese medicine health care literacy questionnaire for the elderly,and the Chinese version of the Diabetes Distress Scale(DDS) were used to investigate patients.And analyzed the influencing factors of frailty in elderly diabetic patients. Results: The total score of weakness among 260 elderly diabetic patients was(7.79±3.92) points,of which 179(68.8%) were weak.The results of linear regression analysis showed that traditional Chinese medicine health care literacy,age,psychological pain,payment method of medical expenses,diabetic complications,drug use,residence,and night sleep time were the influencing factors of frailty in elderly diabetic patients,accounting for 65.0% of the total variation of weakness. Conclusions: Frailty in elderly diabetic patients at the middle level,and nursing staff should develop effective intervention plans based on influencing factors of frailty.

Chinesization of Fear of Activity in Patients with Coronary Artery Disease and its reliability and validity test

Objective: To translate Fear of Activity in Patients with Coronary Artery Disease(Fact-CAD) and test its reliability and validity. Methods: Fact⁃CAD was translation according to the Brislin translation model.Provisional Chinese version of Fact⁃CAD was developed after cultural adaptation and pre-investigation.A total of 340 patients with coronary heart disease hospitalized in cardiovascular department of a tertiary grade A hospital in Luoyang city were selected as research subjects from February 22 to September 30,2021.Item of provisional Chinese version of Fact⁃CAD was analyzed,and reliability and validity was test. Results: The Chinese version of Fact⁃CAD was single dimensional scale,and its Cronbach's α coefficient was 0.982,the half reliability was 0.986, the retest reliability was 0.854,the item⁃level content validity index was from 0.83 to 1.00,scale⁃content validity index was 0.90.Exploratory factor analysis extracted one common factor with an eigenvalue of 15.484,and the variance contribution rate was 73.732%. Conclusions: The Chinese version of Fact⁃CAD has good reliability and validity,and can be used for assessing fear of activity in Chinese patients with coronary heart disease.

Scoping review of frailty risk prediction models

Objective: To systematically integrate the construction and application status,predictors and performance of frailty risk prediction models in China and abroad. Methods: The Chinese and English databases were retrieved,and retrieval time from database establishment to June 30,2022.Data was extracted and summarized for analysis.According to Prediction model Risk Of Bias Assessment Tool,the risk of bias was evaluated from four aspects with research objects,predictor,outcome and analysis of the models established. Results: A total of 12 literature was included in the study,which was mainly carried out by Chinese.The research subjects were mainly elderly people.The model construction methods could be divided into Logistic regression model and machine learning.The model mainly displayed the risk scoring formula based on regression coefficients of each factor.The three predictive factors with the highest frequency of occurrence were comorbidities,age,and multiple drug use. Conclusions: The models included in this study have good prediction,but the overall bias risk of the study is higher.In the future,visual model presentation methods can be applied,and models with low bias risk,good predictive performance,and high clinical practicality can be established.
